FAQ's

How do I make an appointment?

There are several ways to make an appointment:

  • Call 800-511-5446 during office hours (*8am to 6pm Monday through Wednesday, and 8am 3pm Thursday & Friday) and speak directly to one of our Customer Service Reps or come in to an office near you during our open intake hours. *Offices that close at 5pm or are by appointment only, please call 800-511-5446.
     

  • Have a teacher, social worker, or medical person fill out the “agency referral” form on our website, and we’ll call you to schedule an appointment.

Crisis Services

Crisis Services are available 24/7/365 locally and statewide:

  • If you are in crisis, please call 800-511-5446 (Local) or 844-493-8255 (Statewide) or you can text TALK to 38255.
     

  • Southeast Health Group is contracted to provide mental health and substance use crisis assessments to Health First Colorado (Medicaid) clients in Baca, Bent, Crowley, Kiowa, Otero and Prowers counties 24/7/365.
     

  • We have made it part of our mission to serve anyone in crisis and agree to respond to calls for any payer type, including those with third party insurance or no insurance.

Is there an office in my community?

Do you take private insurance?

We accept most private insurances, as well as Health First Colorado (Medicaid) and Medicare.

What Services does SHG provide?

We provide services ranging from Crisis Mental Health, Addiction Therapy to Physical Therapy and many more.

What if I don’t know what the problem is?

Our professional staff are skilled in the assessment and diagnosis of a variety of physical, emotional and substance use disorder concerns.

  • You can discuss what is going on at your first appointment, and we will help direct you to the right treatment professional.

How quickly can I be seen for behavioral health care?

At Southeast Health Group, we do not have a waiting list for behavioral health services.

  • All behavioral health patients are offered an appointment within seven days of the first contact or referral.

  • As always, crisis services are available 24/7/365 for those in crisis Call 800-511-5446.

What if I have a complaint about the SHG services I receive?

Our Advocacy Director is available to listen to any concerns you have about our services and help you resolve the issue.

How do I sign up for Health First Colorado (Colorado's Medicaid) or Private Insurance?

  • SHG is a CAAS (Certified Application Assistance Site). Our Health Navigators will be able to assist you in signing up for Health First Colorado (Colorado's Medicaid). Call 800-511-5446 to arrange getting assistance.

  • You can also contact your local county Department of Human Services.

  • For private insurance options, go to www.connectforhealthco.com.

Intake

All patients, regardless of payer/insurance, are offered an intake appointment within 7 business days.  People with urgent needs are offered a same day, walk-in intake appointment.

  • In many cases, the individual does not accept the first available appointment and therefore it may push them out another week or so until they find an appointment that works with their schedule.
     

  • As always, crisis services are available 24/7/365 for those in crisis. It should be noted that crisis services are not necessarily an intake, but instead an assessment of safety.  An intake would still need to be completed at another time to be enrolled in outpatient services.

At Southeast Health Group, we:

  • Serve all patients regardless of ability to pay  

  • Offer a discounted fee program (sliding fee scale), based on family size and income

  • Accept Medicaid, Medicare, and CHP+

  • Will not deny services based on a patient’s: Race, Sex, Disability, Color, Age, Sexual Orientation, National Origin, Religion, Gender Identity
